I agree to my child(ren) taking part in tennis activities at North Dulwich Lawn Tennis Club. He/she has agreed to follow the junior rules of the club and I agree to accept the code of conduct for parents

I am aware that I am responsible for safely delivering and collecting the child/children under my care from the Club courts and understand the Club cannot be responsible for them once they have left the courts and are out of sight. 

To my knowledge he/she has no special care needs, dietary requirements, allergies or medical conditions that could affect his/her safety at the club, other than those declared when booking. I understand that in the event of an injury, illness or other medical need, all reasonable steps will be taken to contact me and deal with the situation appropriately. I understand that it is my responsibility to inform the club of any changes to the information provided on this form.


CANCELLATIONS AND REFUNDS FOR TERMLY CLASSES

CANCELLING BEFORE A TERM HAS STARTED: If you need to cancel a booking as you can no longer attend a term, you will receive a full refund (less a 5% admin fee) if cancelling at least 5 days before the start of term. If cancelling less than 5 days before the start of term, a refund less the cost of the first lesson (and 5% admin fee) will be processed.

CANCELLING DURING A TERM

If cancelling part way through a term, a refund will be processed for any remaining classes in the term - less a 5% admin fee.
